Allen Park Pre-Enrollment Application for ALLEN PARK RESIDENTS

Welcome to the Allen Park Public School District! To begin the enrollment process, please use the pre-enrollment link. You will be required to upload the following documents:

  • Student Birth Certificate
  • Parent/Guardian ID (driver’s license, state ID, passport, or visa) for identification purposes
  • Proof of Residency - To enroll a student, the State of Michigan requires (2) items.
    • Mortgage, Purchase or Lease Agreement, Property Tax Document
    • One (1) of the following
      1. Gas, Electric, Water, Internet or Cable bill (within the last 30 days addressed to the person enrolling the student)
      2. Home Insurance
  • Most Recent Report Card (applies to 1st - 9th grades)
  • High School Transcript (applies to 10th - 12th grades)
  • Individualized Education Program (IEP), if services are required
  • Proof of Custody/Guardianship - if student is not living with a parent
  • Up-to-date Immunization Record*
    • Incoming Kindergarten and 1st Grades
      • Health Appraisal (including Hearing and Vision Screening)
      • Dental

*A current immunization record, or waiver from the State of Michigan, is required for enrollment to the 2025-26 school year (Enrollments will not be processed without it). An up-to-date immunization record, or waiver from the State of Michigan, is required before your student can attend class in the fall.
